Vision plans are allowances, not percentages
Most vision plans work on a schedule rather than a deductible. You get a covered eye exam for a small copay, then a fixed dollar allowance towards frames and either an allowance or full cover for standard lenses, typically once every twelve or twenty-four months.
Because the structure is so consistent between carriers, comparing plans is mostly a matter of three things: the frame allowance, how often you can use it, and whether your optician is in the network. Contact lens allowances usually replace the glasses benefit rather than adding to it.
What vision plans typically cover
Benefits are usually grouped into four areas, each with its own frequency and allowance.
Routine eye exams
Generally covered once a year for a modest copay. This is the benefit most people actually use, and it catches more than sight problems.
Frame allowance
A fixed dollar amount towards frames, commonly every twelve or twenty-four months. Anything above the allowance you pay yourself, often at a network discount.
Lenses and coatings
Standard single vision, bifocal and trifocal lenses are usually covered. Progressives, anti-glare and photochromic coatings normally cost extra.
Contact lens allowance
An allowance towards contacts, usually instead of the frame benefit in the same period rather than in addition to it.

Common questions
Is vision cover included in health insurance?
For children, routine vision is an essential health benefit on ACA plans. For adults it generally is not, which is why standalone vision plans exist.
How often can I get new glasses?
Most plans allow new lenses every twelve months and new frames every twelve or twenty-four. Check the frequency before assuming you can claim annually.
Does vision insurance cover laser surgery?
Rarely as a benefit. Many plans instead offer a negotiated discount at participating surgeons, which is not the same as cover.
Can I use my allowance anywhere?
Usually only at network providers. Out-of-network benefits, where they exist, are typically reimbursed at a much lower fixed amount.
Is a vision plan worth it for one exam a year?
Sometimes not. If you do not wear corrective lenses, paying cash for an annual exam can cost less than twelve months of premiums. We will tell you if that is your situation.
